Scenic Bluffs, recognized as a patient centered medical home (pcmh) by the national committee for quality assurance, provides primary, preventive healthcare to the underserved populations of monroe, vernon, crawford and la crosse counties in western Wisconsin. Scenic Bluffs works to address the care needs of our patients and communities, focusing on the issues that keep people healthy. We do this through educational, preventionbased programming that connects patients with resources and education. Scenic Bluffs aims to address social determinants of Health through educational, preventionbased programming that connects patients with resources and education. This is possible through our Health educators, help team, certified application counselor, and our engagement in nearly 50 community coalitions and boards.
